This practical course gives learners a structured approach to investigating workplace accidents, incidents, and near misses. Learners gather evidence, identify immediate and underlying causes, and develop actions to prevent recurrence.
Effective investigation helps organisations understand what happened, why it happened, and what must change to prevent recurrence. The course follows the HSE HSG245 investigation process: gather information, analyse the information, identify risk-control measures, and prepare and implement an action plan. It also explains the relationship between internal reporting, record keeping, and RIDDOR.
A range of subjects are covered including:
Understanding why accidents, incidents, and near misses should be reported and investigated, and how learning from events supports prevention and continual improvement.
Taking immediate action following an event, including first aid, making the area safe, protecting people, preserving evidence, and escalating serious incidents appropriately.
Planning an investigation, defining the scope, selecting a proportionate investigation level, and involving managers, workers, safety representatives, and technical specialists where required.
Gathering reliable information through scene examination, photographs, documents, equipment records, procedures, training records, timelines, and structured witness interviews.
Distinguishing facts from assumptions and analysing immediate, underlying, and root causes, including human, task, equipment, environmental, and organisational factors.
Identifying practical risk-control measures using the hierarchy of control, allocating responsibilities and timescales, and tracking actions through to effective completion.
Producing a clear investigation report, sharing lessons learned, reviewing risk assessments and procedures, and understanding when an event may require recording or reporting under RIDDOR.
Completing a practical investigation case study and presenting evidence-based findings and recommendations.
Assessment is ongoing through learner participation, questioning, and completion of the practical investigation case study. There is no formal grading; learners pass or are referred.
A certificate of attendance is issued on successful completion. Refresher training should be considered when responsibilities, reporting requirements, workplace activities, or organisational procedures change.
